Bert Formanik, age 94. died on November 13, 2016.

He was the beloved husband of the late Virginia (nee Zyck) and the loving father of Karen Formanik-Wells (Stanley), Ronald (Vicki), John and James (Susan).

He was the brother of the late Phyllis Sobina, Walter Foreman, John and Edmund.

Bert was also survived by his grandchildren, Timothy and Jenna Wells, Lisa, Kristie, Chelsea, Michael, Melissa Heinrich (Chris) and Angela.

Also survived by nieces, nephews, and many dear friends.

U.S. Air Force World War II Veteran.
