According to police, around 8:40 a.m. on Wednesday, December 5, 69-year-old Charles E. Samuels Jr., of Diamond, Ohio, was operating a 2018 Mack Truck, traveling north on State Route 66, just south of Blue Jay Creek Road, in Howe Township, Forest County.
Police say Samuels failed to stop for a 2017 Hyundai Elantra, driven by 61-year-old Barry W. Farmer of Irwin, Pa., which was making a left turn onto Blue Jay Creek Road from State Route 66 and struck Farmer’s vehicle from behind.
After the initial impact, Farmer’s vehicle continued off the east side of the roadway and struck multiple large rocks head-on.
Both vehicles then came to a final rest off the east side of the roadway.
Both drivers were wearing seatbelts.
Farmer suffered suspected minor injuries and was transported to Clarion Hospital by Clarion Hospital Ambulance for further evaluation.
Samuels suffered minor injuries but refused transportation to the hospital.
Samuels was charged with multiple traffic violations.
Marienville Volunteer Fire Company and Frank’s Towing assisted at the scene.
